Summary

Polypoidal choroidal vasculopathy (PCV) affects nearly 10% of elderly Italians with macular exudation. Suspect PCV in patients with extramacular lesions and minimal fellow eye drusen.

Area of Science:

  • Ophthalmology
  • Medical Research
  • Retinal Diseases

Background:

  • Age-related macular degeneration (ARMD) is a leading cause of vision loss in the elderly.
  • Distinguishing ARMD from polypoidal choroidal vasculopathy (PCV) is crucial for appropriate treatment.
  • Macular exudation in elderly patients necessitates careful etiological investigation.

Purpose of the Study:

  • To determine the prevalence and clinical characteristics of PCV in Italian elderly patients with macular exudation.
  • To compare the features of PCV with ARMD complicated by choroidal neovascularization.

Main Methods:

  • Retrospective analysis of 194 consecutive patients aged 50+ with exudative maculopathy.
  • Diagnostic tools included color/red-free fundus photography and fluorescein/indocyanine green angiography.

Related Experiment Videos

Main Results:

  • Polypoidal choroidal vasculopathy (PCV) was diagnosed in 9.8% of patients.
  • PCV cases were more frequently unilateral (78.9%) compared to ARMD (58.9%).
  • Extramacular choroidal neovascularization was significantly more common in PCV (47.3%) than ARMD (2.9%).

Conclusions:

  • Polypoidal choroidal vasculopathy is a notable condition in the Italian elderly population.
  • Clinical suspicion for PCV is warranted in cases with extramacular lesions and absent significant fellow eye drusen.
