Area of Science:

  • Urology
  • Trauma Surgery
  • Psychiatry

Background:

  • A 52-year-old male bag craftsman presented with a significant scrotal hematoma following direct trauma to the right testis.
  • The patient self-treated the severe pain by resecting his right testis with scissors approximately 30 minutes after the injury.

Observation:

  • The patient sutured the scrotal incision himself with two stitches.
  • He presented to the hospital 36 hours after the self-mutilation with a large scrotal hematoma.

Findings:

  • Emergency surgery revealed a 283g hematoma, which was evacuated.
  • The cut end of the right spermatic cord was ligated after thorough debridement.
  • Psychiatric evaluation indicated mild intellectual disability without psychosis or substance abuse.

Implications:

  • This case highlights a rare instance of self-mutilation following testicular trauma.
  • It underscores the importance of prompt medical evaluation for scrotal injuries, even after self-treatment.
  • The psychological assessment is crucial in managing patients with self-inflicted injuries.
