Purpose of the Study:

  • To compare the predictive validity of demographic and historical risk factors against the dexamethasone suppression test (DST) for suicide risk in major depressive disorder.
  • To assess the DST as a measure of hypothalamic-pituitary-adrenal (HPA) axis hyperactivity for suicide risk prediction.

Main Methods:

  • Seventy-eight inpatients diagnosed with major depressive disorder or schizoaffective disorder, depressed type, were included.
  • Patients underwent a 1-mg DST, and suicide rates were tracked over a 15-year follow-up period.
  • The predictive validity of four demographic/historical factors was compared with DST results.

Main Results:

  • Abnormal DST results were observed in 32 out of 78 patients.
  • Patients with abnormal DST results had an estimated 26.8% risk of eventual suicide, versus 2.9% for those with normal DST results.
  • Demographic and historical risk factors did not significantly predict suicide in this cohort.

Conclusions:

  • Hypothalamic-pituitary-adrenal (HPA) axis hyperactivity, indicated by DST results, may be a more powerful predictor of suicide risk than current clinical tools.
  • Future research should focus on the HPA axis and its interaction with the serotonin system in the pathophysiology of suicidal behavior in major depressive disorder.
