Summary

Phacoemulsification (PEA) using a sleeveless ultrasound tip in ultrasmall incisions does not cause thermal burns when sufficient irrigation is maintained. This technique is safe for cataract surgery.

Area of Science:

  • Ophthalmology
  • Surgical Technology

Context:

  • Phacoemulsification (PEA) is a common cataract surgery technique.
  • Ultrasmall incisions are increasingly used to improve surgical outcomes.
  • The use of an ultrasound tip without an infusion sleeve raises concerns about thermal safety.

Purpose:

  • To investigate the thermal effects of using an ultrasound tip without an infusion sleeve during ultrasmall incision phacoemulsification.
  • To determine the risk of thermal injury to the incision wound under these conditions.

Summary:

  • The study observed ultrasound tip temperatures and measured wound temperatures in porcine eyes during simulated PEA with a sleeveless 20-gauge tip.
  • No significant temperature rise at the ultrasound tip occurred with adequate fluid circulation.
  • No thermal burns were observed in wounds wider than 1.5 mm, indicating safety.

Related Experiment Videos

Impact:

  • Demonstrates the safety of performing ultrasmall incision cataract surgery with a sleeveless ultrasound tip.
  • Provides evidence that adequate irrigation prevents thermal damage during PEA.
  • Supports the adoption of this technique for potentially improved patient recovery.
