Area of Science:

  • Hepatology
  • Pharmacology
  • Clinical Medicine

Background:

  • Recurrent sinusitis necessitates antibiotic treatment.
  • Fluoroquinolone antibiotics, including trovafloxacin, are used for bacterial infections.
  • Drug-induced liver injury (DILI) is a significant concern in clinical practice.

Observation:

  • A 19-year-old woman presented with acute hepatitis, peripheral eosinophilia, hepatosplenomegaly, and ascites.
  • These symptoms emerged during treatment for recurrent sinusitis with oral trovafloxacin.
  • Laparoscopic liver biopsy revealed extensive centrilobular hepatocyte necrosis.

Findings:

  • The clinical presentation and biopsy findings were consistent with severe drug-induced liver injury.
  • The temporal association strongly implicated trovafloxacin as the causative agent.
  • Complete resolution of hepatic abnormalities occurred following prolonged corticosteroid therapy.

Implications:

  • This case highlights the potential hepatotoxicity of trovafloxacin.
  • Prompt recognition and cessation of the offending drug, along with appropriate supportive care (e.g., steroids), are crucial for managing DILI.
  • Further vigilance is warranted when prescribing fluoroquinolones, especially in patients with pre-existing liver conditions or risk factors.
