Area of Science:

  • Otology
  • Neurology
  • Vestibular System

Background:

  • Meniere's disease (MD) is a disorder of the inner ear.
  • Tumarkin attacks, also known as drop attacks (DA), are characterized by sudden loss of balance with preserved consciousness.
  • DA are hypothesized to be linked to otolith dysfunction in MD.

Purpose of the Study:

  • To investigate the prevalence and characteristics of drop attacks (DA) in patients with Meniere's disease (MD).
  • To explore potential triggers and associated symptoms of DA in MD patients.

Main Methods:

  • Data from 243 consecutive Meniere's disease patients were analyzed.
  • An otoneurologic expert system database was utilized.
  • Comparison between patients experiencing DA and those who did not.

Main Results:

  • Drop attacks (DA) were experienced by 72% of Meniere's disease (MD) patients.
  • DA correlated with visually provoked vertigo.
  • Gait difficulties, tinnitus, anxiety, and specific triggers (physical strain, head movements, pressure changes, visual stimuli) were more prevalent in the DA group.

Conclusions:

  • Drop attacks (DA) are a common and poorly tolerated symptom in Meniere's disease (MD) patients.
  • DA are more frequently observed in advanced stages of MD.
