Summary

Incomplete left atrial appendage ligation during heart surgery may not prevent stroke. In some cases, incomplete closure might even increase the risk of blood clots forming and traveling to the brain.

Background:

  • The left atrial appendage (LAA) is a common source of thromboemboli in patients with mitral valve disease.
  • Ligation of the LAA is a standard surgical procedure during mitral valve surgery to mitigate stroke risk.

Observation:

  • A case study of a patient experiencing a stroke was analyzed.
  • The stroke was found to be caused by emboli originating from an incompletely ligated LAA.

Findings:

  • Suture ligation of the LAA may be ineffective in preventing thromboemboli.
  • Incomplete LAA exclusion might paradoxically increase the risk of stroke.

Implications:

  • Current LAA ligation techniques may require re-evaluation for efficacy in stroke prevention.
