Area of Science:

  • Neurosurgery
  • Oncology

Background:

  • Intradural spinal metastases are rare but associated with poor outcomes.
  • Optimal treatment strategies for intradural spinal metastases remain controversial.

Purpose of the Study:

  • To evaluate the clinical outcome and surgical treatment of spinal metastases, with a focus on intradural cases.

Main Methods:

  • Retrospective review of 220 spinal metastases treated between 1980 and 1999.
  • Detailed analysis of 11 intradural metastases, including clinical history, signs, surgical approach, and outcomes.

Main Results:

  • Secondary spinal tumors commonly originated from breast, lung, kidney, gastrointestinal, and prostate carcinomas.
  • Intradural metastases (melanoma, lymphoma, medulloblastoma) showed worse functional recovery and survival than extradural ones.
  • Patients with intradural metastases were generally younger.

Conclusions:

  • Intradural spinal metastases represent a devastating condition with a typically fatal outcome.
  • Palliative treatment aims to relieve pain and preserve neurological function.
  • Selected patients undergoing aggressive surgery may experience significantly prolonged survival.
