Summary

Candiduria, or Candida in the urine, is rising. While fluconazole is common, optimal treatment for this fungal urinary tract infection requires more research on diagnosis and patient-specific approaches.

Area of Science:

  • Mycology
  • Infectious Diseases
  • Urology

Background:

  • Fungal urinary tract infections, particularly candiduria, are increasingly prevalent.
  • Distinguishing Candida colonization from infection is challenging, often relying on pyuria and high urine Candida counts.
  • Current treatment guidelines for candiduria remain largely undefined.

Purpose of the Study:

  • To review the current diagnostic criteria for candiduria.
  • To summarize established and emerging treatment strategies for fungal urinary tract infections.
  • To highlight areas requiring further research in candiduria management.

Main Methods:

  • Literature review of studies on candiduria diagnosis and treatment.
  • Analysis of treatment approaches based on infection site (cystitis vs. pyelonephritis).

Related Experiment Videos

  • Synthesis of current understanding of antifungal therapy, including fluconazole.
  • Main Results:

    • Asymptomatic nosocomial candiduria generally has low morbidity and rarely requires intervention.
    • Candida cystitis is typically treated with oral fluconazole.
    • Ascending pyelonephritis necessitates systemic antifungals and potential intervention for obstruction.

    Conclusions:

    • Further research is crucial to establish clear diagnostic criteria and therapeutic pathways for candiduria.
    • Treatment decisions for fungal urinary tract infections must consider the specific clinical context and anatomic site.
    • Optimizing the management of candiduria requires a better understanding of when and how to treat.
