This case report details a rare instance of subacute endophthalmitis caused by Bacillus circulans following cataract surgery. The patient fully recovered with posterior capsulectomy and antibiotic treatment.

Background:

  • Post-cataract surgery endophthalmitis is a serious ocular infection.
  • Bacillus species are known causes of bacterial endophthalmitis, often presenting aggressively.
  • Chronic endophthalmitis typically presents with white plaques on the posterior capsule, often associated with Propionibacterium acnes.

Observation:

  • An 80-year-old woman developed endophthalmitis 17 months after uncomplicated cataract surgery.
  • Clinical presentation included chalky white deposits on the posterior capsule.
  • A three-port pars plana vitrectomy and partial posterior capsulectomy were performed.

Findings:

  • Culture of the removed material identified Bacillus circulans.
  • The patient experienced a rapid and complete recovery with topical cephalothin and prednisolone acetate.
