Summary

Background:

  • Persistent polyclonal B-cell lymphocytosis (PPBL) is a rare B-cell lymphoproliferative disorder.
  • The etiology and pathogenesis of PPBL remain incompletely understood.

Observation:

  • A familial cluster of PPBL was observed in a brother and sister exhibiting characteristic morphological and immunophenotypic features.
  • Both affected siblings presented with mild splenomegaly, elevated serum IgM levels, and serological evidence of prior Epstein-Barr virus (EBV) infection.
  • Two unaffected brothers shared similar HLA haplotypes, smoking habits, and EBV infection status, indicating that these factors alone are insufficient for PPBL development.
