Summary

Partial liquid ventilation (PLV) with perflubron shows dose-dependent radiographic filling patterns. Lower doses may cause inhomogeneous lung filling and temporary oxygenation issues in acute lung injury patients.

Background:

  • Partial liquid ventilation (PLV) using perflubron is an experimental lung protective strategy.
  • Acute lung injury (ALI) is characterized by diffuse bilateral infiltrates and impaired gas exchange.
  • Evaluating the radiographic patterns of PLV is crucial for understanding its efficacy and safety.

Observation:

  • Chest radiographs of 16 ALI patients receiving perflubron PLV were reviewed.
  • Patients received either a 10-mL/kg or 20-mL/kg loading dose of perflubron.
  • Radiographic filling patterns were assessed in relation to dose and clinical variables.

Findings:

  • Inhomogeneous radiographic filling occurred in 5 patients within the first 48 hours.
  • This pattern was significantly associated with the lower loading dose (10 mL/kg) of perflubron.
