Infliximab or biosimilars in sarcoidosis; to switch or not to switch?

Background:

  • Sarcoidosis involves inflammation driven by cytokines like tumor necrosis factor-alpha (TNF-alpha).

Purpose of the Study:

  • To evaluate the efficacy of infliximab in treating chronic, resistant sarcoidosis.
  • To assess infliximab's impact on specific sarcoidosis manifestations.

Main Methods:

  • Three patients with persistent, symptomatic sarcoidosis were enrolled.
  • Infliximab was administered at a dose of 5 mg/kg at weeks 0, 2, 4, and 12.
  • Clinical outcomes, including index lesions and vital capacity, were assessed at 16 weeks.

Main Results:

  • Two patients with lupus pernio experienced significant lesion improvement.
  • One patient with restrictive lung disease showed a 26% increase in vital capacity.
