Summary

Recently described hyperplastic mesothelial cells in lymph nodes may arise from serosal effusions. This case links intraperitoneal hemorrhage and ascites to mesothelial cell dissemination in lymph node sinuses.

Background:

  • The concurrent presence of benign sinus mesothelial cells and neoplastic processes creates unusual lymph node architectural patterns.

Observation:

  • A young male patient presented with diffuse metastases from a primary cardiac angiosarcoma.
  • Autopsy revealed periaortic lymph nodes with metastatic angiosarcoma and hyperplastic mesothelial cells in a sinus distribution.
  • The patient had a history of progressive hemoperitoneum, thrombocytopenia, and disseminated intravascular coagulation, with 5000 ml of hemoperitoneum confirmed at autopsy.
