    Summary

This study analyzed 151 patients with chronic monarthritis, finding synovitis of unknown cause was most common. Most patients, excluding rheumatoid arthritis cases, experienced favorable prognoses with improvement or remission.

    Area of Science:

    • Rheumatology
    • Clinical Medicine

    Background:

    • Monarthritis, defined as arthritis affecting a single joint for over three months, often involves the knee.
    • Understanding the causes and prognoses of chronic monarthritis is crucial for effective patient management.

    Purpose of the Study:

    • To analyze the etiological spectrum and clinical outcomes of chronic monarthritis.
    • To differentiate between various causes of monarthritis and assess their respective prognoses.

    Main Methods:

    • Retrospective review of 151 patients diagnosed with monarthritis exceeding three months' duration.
    • Classification of patients based on etiological diagnosis, including synovitis of unknown cause, osteoarthrosis-associated synovitis, and rheumatoid arthritis.

    Main Results:

    • Synovitis of unknown cause was the most frequent diagnosis (32%), followed by synovitis associated with osteoarthrosis (29%).
    • Rheumatoid arthritis accounted for 9% of cases, with most developing polyarticular involvement.
    • Tuberculous synovitis was rare, with only one case identified.

    Conclusions:

    • Chronic monarthritis has a diverse etiology, with a significant proportion of cases being idiopathic synovitis.
    • Prognosis for most monarthritis cases, particularly those of unknown cause or associated with osteoarthrosis, is generally favorable.
    • Rheumatoid arthritis, though less common in this cohort, follows a more aggressive polyarticular course.
