Nalidixic acid use during pregnancy may slightly increase the risk of congenital abnormalities. A higher prevalence of pyloric stenosis was observed in infants whose mothers took nalidixic acid late in pregnancy.

  • Quinolones, particularly nalidixic acid, are commonly prescribed for bacterial infections in pregnant women in Hungary.
  • No prior controlled epidemiological studies have investigated congenital abnormalities in infants exposed to nalidixic acid during pregnancy.

Purpose of the Study:

  • To investigate the association between nalidixic acid exposure during pregnancy and the occurrence of congenital abnormalities in offspring.
  • To assess the risk of specific congenital abnormalities following maternal treatment with nalidixic acid.

  • A population-based case-control study utilizing the Hungarian Case-Control Surveillance of Congenital Abnormalities dataset (1980-1996).
  • Inclusion of 22,865 cases with congenital abnormalities and 38,151 population-based controls without congenital abnormalities.

  • Main Results:

    • Overall nalidixic acid use was similar between cases (1.1%) and controls (1.0%), with a crude odds ratio of 1.1 (95% CI: 0.9-1.3).
    • A statistically significant higher prevalence of pyloric stenosis was found in infants exposed to nalidixic acid during the last trimester of pregnancy (adjusted OR: 11.0, 95% CI: 1.3-91.4).
    • Seventeen different congenital abnormality groups were evaluated.

    Conclusions:

    • Maternal treatment with nalidixic acid during pregnancy may be associated with an increased risk of pyloric stenosis in infants.
    • While the association is noted, the possibility of chance effects cannot be entirely ruled out due to the observed confidence interval for pyloric stenosis.
