Background:

  • Penetrating trauma to the lower limb can result in significant vascular injury.
  • Initial assessment of vascular integrity may not always reveal subtle injuries.

Observation:

  • A patient presented with a lower limb injury with minimal initial signs of vascular damage.
  • Ankle systolic pressure was normal on initial evaluation.

Findings:

  • A pseudoaneurysm of the superficial femoral artery was diagnosed 14 days post-injury.
  • An arteriovenous fistula in the left thigh was concurrently identified.

Implications:

  • A high index of clinical suspicion is vital for detecting missed vascular injuries.
  • Thorough clinical review is essential to prevent delayed diagnosis of vascular complications.
  • Prompt diagnosis and management of vascular trauma can prevent severe patient outcomes.
