Sexual behavior in sleep, sleepwalking and possible REM behavior disorder: a case report

This case report details a man with a history of sleepwalking who developed adult-onset sexual behavior during sleep (SBS) and violent nocturnal episodes. Treatment with clonazepam led to significant clinical improvement.
Area of Science:
- Neurology
- Sleep Medicine
- Psychiatry
Background:
- Parasomnias, including sleepwalking (SW), are disruptive sleep disorders.
- Sexual behavior during sleep (SBS) is a rare parasomnia, often associated with other sleep disorders.
- Parasomnia overlap syndrome involves the co-occurrence of multiple parasomnias.
Observation:
- A 27-year-old male presented with a history of childhood sleepwalking and adult-onset violent nocturnal behaviors with dream enactment.
- The patient also experienced amnestic sexual behavior episodes during sleep, including sexual intercourse.
- Family history revealed sleepwalking in his brother, and diagnostic workups were unremarkable.
Findings:
- The case suggests a potential combination of violent/non-violent sleepwalking with SBS.
- Alternatively, it may represent REM sleep behavior disorder (RBD) superimposed on pre-existing SW, with subsequent SBS development.
- This presentation is consistent with probable parasomnia overlap syndrome.
Implications:
- This case highlights the complex presentation of parasomnias, particularly the overlap of SW, RBD, and SBS.
- Effective management with clonazepam suggests a potential therapeutic avenue for similar complex cases.
- Further research is needed to understand the underlying mechanisms and genetic predispositions of parasomnia overlap syndrome.
