Summary

Bone block procedures are effective for unidirectional posterior shoulder instability without additional laxity. However, they show a high recurrence rate for posterior instability with additional laxity, indicating limited efficacy in complex cases.

Background:

  • Recurrent posterior shoulder instability often requires surgical intervention when conservative treatments fail.
  • Bone block procedures are utilized for shoulder instability, but their efficacy in posterior instability requires further evaluation.

Observation:

  • This study evaluated eleven shoulders in ten patients undergoing a bone block procedure for recurrent posterior shoulder instability.
  • Patients were categorized into two groups: unidirectional posterior instability (UPI-) and posterior instability with additional laxity (UPI+).

Findings:

  • The UPI- group demonstrated a 100% success rate with no recurrence of posterior instability after a median follow-up of 72 months.
  • The UPI+ group experienced a high recurrence rate, with only a 20% success rate, suggesting the bone block procedure is insufficient for this subgroup.
