Summary

This study details a rare case of basal cell carcinoma (BCC) co-occurring with multiple trichoepitheliomas (TEs). Immunohistochemical analysis suggests a shared origin, with BCC potentially differentiating from follicular germinative cells, the benign counterpart of TEs.

Background:

  • Basal cell carcinoma (BCC) and trichoepitheliomas (TEs) are distinct skin neoplasms.
  • Understanding the relationship between BCC and TEs can provide insights into follicular differentiation pathways.

Observation:

  • A patient presented with a central facial pigmented nodule diagnosed as BCC and multiple flesh-colored papules diagnosed as TEs.
  • Histopathological examination revealed BCC and TEs in the excised lesions.

Findings:

  • Immunohistochemical staining showed diffuse positivity for cytokeratin (CK) 19, CK8, and CK8/18 in the basaloid aggregations of both BCC and TEs.
  • These findings suggest a close clinicopathological relationship between BCC and TEs.
