    Left ventricular ejection fraction (LVEF) is typically normal in severe chronic obstructive airways disease (COPD) patients without coronary artery disease. Reduced LVEF in COPD patients is likely due to coexisting coronary artery disease, not lung disease alone.

    Background:

    • The relationship between chronic obstructive airways disease (COPD) and left ventricular function remains a subject of debate.
    • Assessing left ventricular ejection fraction (LVEF) is crucial for understanding cardiac health in patients with respiratory conditions.

    Purpose of the Study:

    • To investigate left ventricular ejection fraction (LVEF) in patients with severe chronic obstructive airways disease (COPD).
    • To determine if COPD itself or coexisting coronary artery disease (CAD) affects LVEF.

    Main Methods:

    • Measured LVEF using a bedside radionuclide technique with a scintillation probe in 120 severe COPD patients (92 acute, 28 stable).
    • Assessed for the presence of coronary artery disease (CAD) clinically or via autopsy.

    • No correlation was found between LVEF and arterial oxygen, carbon dioxide tension, or pH.

    Conclusions:

    • Severe chronic obstructive airways disease (COPD) alone does not appear to impair left ventricular ejection fraction (LVEF).
    • Reduced LVEF in patients with COPD is strongly associated with the presence of coronary artery disease (CAD).
    • Clinical evaluation for CAD is essential in COPD patients presenting with diminished LVEF.
