Summary

Childhood multiple sclerosis can present as encephalopathy with consciousness disturbance. Lesions in the brainstem activating the reticular formation, including the thalamus, likely cause these symptoms.

Background:

Observation:

  • A nine-year-old girl presented with consciousness disturbance, drowsiness, emotional instability, decreased vision, and coordination issues.
  • Cerebrospinal fluid showed elevated IgG and myelin basic protein.
  • EEG revealed frontal slow waves; MRI identified demyelinating plaques in the brainstem, thalamus, and white matter, involving the reticular formation.

Findings:

  • The patient had a history of bilateral optic neuritis and papillitis, supporting the MS diagnosis.
  • Methylprednisolone pulse therapy led to significant improvement in neurological symptoms and MRI findings.
