    Background:

    • Transitional cell carcinoma of the bladder is a common malignancy.
    • Cisplatinum is a widely used chemotherapeutic agent.
    • Understanding drug distribution is crucial for optimizing cancer therapy.

    Purpose of the Study:

    • To quantify cisplatinum levels in various bladder tissues after intravesical administration.
    • To compare cisplatinum distribution in normal bladder tissue, tumor tissue, inflamed mucosa, and blood.
    • To evaluate the differential uptake of cisplatinum by tumor versus normal bladder tissue.

    Main Methods:

    • Tissue samples from 32 bladder cancer patients were analyzed.
    • Neutron activation analysis using an LP-4900 analyzer with a semiconductor radiation detector was employed.
    • Samples were subjected to radiochemical purification prior to platinum assay.

    Main Results:

    • Intravesical cisplatinum administration led to 33.7 times higher platinum concentrations in tumor tissue than in normal bladder tissue.
    • Following intravenous administration, normal and tumor bladder tissues showed similar cisplatinum quantities.
    • Tumor tissue demonstrated significantly greater absorption of cisplatinum compared to normal bladder tissue after intravesical delivery.

    Conclusions:

    • Intravesical administration of cisplatinum results in preferential accumulation within bladder tumor tissue.
    • This targeted delivery suggests a potential advantage for intravesical cisplatinum in treating bladder cancer.
    • Further research into optimizing intravesical chemotherapy delivery is warranted.
