A rare case of multiple myeloma with extramedullary plasmacytoma presented as back pain and radiculopathy. The patient was refractory to chemotherapy, developed infections, and ultimately died.

Background:

  • Multiple myeloma (MM) is a plasma cell malignancy.
  • Extramedullary plasmacytoma (EMP) is rare in MM, especially at initial presentation.
  • Spinal involvement can cause significant neurological deficits.

Observation:

  • A 45-year-old female presented with low back pain and L5-S1 radiculopathy.
  • Neuroimaging revealed an epidural tumor compressing the spinal root.
  • No vertebral involvement or osteolytic lesions were observed.

Findings:

  • The patient was diagnosed with immunoglobulin G, kappa multiple myeloma with an extramedullary plasmacytoma.
  • Pathology confirmed high-grade plasmablastic myeloma.
  • The patient showed refractoriness to chemotherapy and progressive renal dysfunction.
