Summary

Acute optic neuritis (ON) can indicate multiple sclerosis (MS). Early diagnosis and treatment with disease-modifying therapy, like interferon-beta-1a, can reduce the progression to clinically definite MS (CDMS).

Area of Science:

  • Neurology
  • Ophthalmology

Background:

  • Optic neuritis (ON) is frequently the initial symptom of multiple sclerosis (MS).
  • Distinguishing between ON patients who will develop MS and those who will not is increasingly critical due to informed patients and emerging MS therapies.

Purpose of the Study:

  • To review the association between optic neuritis (ON) and multiple sclerosis (MS).
  • To highlight the importance of early diagnosis and intervention for patients presenting with ON.

Main Methods:

  • Review of existing literature and clinical trial data.
  • Analysis of a randomized clinical trial involving patients with ON suggestive of MS and subclinical demyelination.

Main Results:

  • Multiple sclerosis (MS) often presents as acute unilateral optic neuritis (ON).
  • A significant proportion of ON patients develop clinically definite MS (CDMS).
  • Treatment with recombinant interferon-beta-1a reduced the development of CDMS in a trial of patients with ON and subclinical demyelination.

Conclusions:

  • Ophthalmologists should refer patients with acute ON to neurologists for evaluation.
  • Early investigations for MS and consideration of disease-modifying therapy are recommended for patients with ON.
