A rare case of acute open dorsal perilunate dislocation, a severe left hand injury exposing the lunate and distal radius, was successfully treated with open reduction. This unique injury pattern involving open dislocation and triquetrum fracture has not been previously documented.

Successful anatomical reduction of the perilunate dislocation was achieved.
