Summary

Hormone replacement therapy with estrogen and progesterone may trigger thyroid-associated orbitopathy (TAO) in postmenopausal women. Discontinuation of these hormones resolved TAO symptoms, suggesting a potential causal link.

Background:

  • Thyroid-associated orbitopathy (TAO) is an autoimmune condition affecting the eye socket.
  • The role of exogenous hormones in TAO pathogenesis is not well-established.
  • Estrogen and progesterone are commonly used in hormone replacement therapy (HRT).

Observation:

  • A 56-year-old postmenopausal woman developed TAO symptoms during estrogen/progesterone therapy.
  • Eye symptoms resolved upon HRT withdrawal and recurred upon re-initiation.
  • The patient had no prior history of thyroid or autoimmune diseases.

Findings:

  • Estrogen and/or progesterone therapy appears to be linked to the development of TAO.
  • Hormone replacement therapy may induce or exacerbate TAO in susceptible individuals.
