Area of Science:

  • Spine surgery
  • Orthopedics
  • Radiology

Background:

  • Spondylolisthesis etiology is diverse, including structural abnormalities like tumors and Paget's disease.
  • Osteoporosis has not been previously recognized as a cause of pathologic spondylolisthesis.

Observation:

  • A case of de novo spondylolisthesis at L5-S1 is presented.
  • The condition developed gradually over three years due to pars interarticularis elongation.

Findings:

  • This case demonstrates osteoporosis as a novel etiology for spondylolisthesis.
  • Radiographic and clinical data support the diagnosis.

Implications:

  • Recognizing osteoporotic spondylolisthesis is crucial, especially in elderly populations with high osteoporosis prevalence.
  • This finding expands the differential diagnosis for pathologic spondylolisthesis.
