Summary

Small intestinal submucosa (SIS) patches show promise for small bowel regeneration, demonstrating successful tissue integration. However, tubular SIS grafts for segmental replacement are currently not feasible due to leakage complications.

Background:

  • Small intestinal submucosa (SIS) is a versatile extracellular matrix utilized in various tissue engineering applications.
  • SIS has been explored for creating abdominal walls, urinary bladders, tendons, blood vessels, and dura mater.

Purpose of the Study:

  • To assess the feasibility of using SIS as a scaffold for small bowel regeneration.
  • To evaluate SIS in an in situ xenograft model for intestinal repair.

Main Methods:

  • Partial small bowel defects in 23 dogs were repaired using SIS patches.
  • Four dogs received tubular SIS grafts for segmental small bowel replacement.
  • Animals were monitored for 2 weeks to 1 year post-implantation.

  • 20 out of 23 dogs with SIS patches survived without intestinal dysfunction, showing no stenosis at necropsy.
  • Histology revealed regenerated mucosal epithelium, smooth muscle, collagen, and serosa, though submucosal organization was poor.
  • Early postoperative leakage caused mortality in 3 dogs with patches and 3 dogs with tubular grafts.
  • One dog with a tubular graft experienced partial obstruction.

Conclusions:

  • SIS patches represent a viable option for small bowel regeneration.
  • Current limitations prevent the use of tubular SIS grafts for segmental small bowel replacement.
