Multicentric osteosarcoma, a rare bone cancer variant, can present with sacral radiculopathy in children. This case highlights unusual findings and the need for broader differential diagnoses in pediatric low back pain.

Background:

  • Multicentric osteosarcoma is an exceptionally rare variant of osteosarcoma.
  • Previously, no reported cases have documented multicentric osteosarcoma presenting as sacral radiculopathy.

Observation:

  • A 14-year-old boy presented with symptoms mimicking a herniated nucleus pulposus.
  • A large sacral tumor extending into the spinal canal was identified as the cause.
  • A bone survey revealed additional multicentric osteosarcoma foci in the long bones.

Findings:

  • The patient received chemotherapy for the diagnosed multicentric osteosarcoma.
  • Despite treatment, the patient succumbed to the disease eight months post-presentation.

  • Multicentric osteosarcoma should be included in the differential diagnosis for pediatric patients experiencing low back pain and sciatica.
  • Early recognition of this rare condition is crucial for timely intervention and management.
