Study Designs in Epidemiology01:20

Study Designs in Epidemiology

Epidemiological study designs are fundamental tools for investigating the distribution, determinants, and control of health conditions in populations. They help researchers understand the relationships between exposures and outcomes, and they broadly fall into two categories: "observational" and "experimental" studies.
Observational studies are those where the researcher does not intervene but rather observes natural variations. They include cross-sectional, cohort, and case-control studies.

Summary

High infant mortality rates, particularly Sudden Infant Death Syndrome, impact Northern Plains Indians. A collaborative study investigated causes and contributing factors to reduce infant deaths.

Area of Science:

  • Public Health
  • Epidemiology
  • Pediatrics

Background:

  • The Aberdeen Area of the Indian Health Service exhibits the highest infant mortality rate in the U.S.
  • Sudden Infant Death Syndrome (SIDS) accounts for half of infant deaths, a rate 4-5 times the national average among certain tribes.

Purpose of the Study:

  • Investigate high infant mortality rates in the Aberdeen Area.
  • Refine cause-of-death ascertainment and improve cause-specific infant mortality rates.
  • Identify contributing factors to elevated infant mortality among Northern Plains Indians.

Main Methods:

  • A 4-year prospective case-control study involving 10 tribes.
  • Data collection included parental interviews, death scene investigations, autopsies, and medical record reviews.
  • Utilized a surveillance system for infant deaths and blood cotinine assays.

Main Results:

  • 72 infant deaths were investigated between December 1992 and November 1996.
  • The study examined infant care practices, socio-demographic, economic, medical, healthcare, and environmental factors.
  • Data analysis is ongoing; results will be published subsequently.

Conclusions:

  • A multi-agency and community-involved model was developed to study infant mortality.
  • Understanding contributing factors is crucial for targeted interventions.
  • Further analysis will provide insights into reducing infant mortality in this population.
