Summary

Rectus sheath hematomas (RSHs) are rare but should be considered for acute abdominal pain in late pregnancy or postpartum. Imaging like sonography and CT can diagnose these hematomas.

Background:

  • Spontaneous rectus sheath hematomas (RSHs) are uncommon during pregnancy.
  • Acute abdominal pain in late pregnancy or postpartum warrants consideration of RSHs.

Observation:

  • A case of pregnancy-associated RSH presented with acute abdominal pain.
  • Initial differential diagnoses included degenerating leiomyoma or torsed ovary.

Findings:

  • Sonography revealed a large, non-vascularized mass of mixed echogenicity.
  • Computed Tomography (CT) confirmed the lesion was located superficially within the rectus sheath.

Implications:

  • Early consideration of RSHs in pregnant patients with abdominal pain is crucial.
