Summary

This study presents a rare case of ethmoidal sinus leiomyosarcoma in a 68-year-old female. Complete endoscopic resection followed by radiotherapy achieved tumor control without recurrence for 29 months.

Background:

  • Paranasal sinus leiomyosarcomas are exceptionally rare malignant neoplasms.
  • Early diagnosis and effective treatment are crucial for managing these tumors.

Observation:

  • A 68-year-old female presented with a six-month history of nasal obstruction and recurrent epistaxis.
  • CT imaging revealed a tumor within the ethmoidal sinus.

Findings:

  • The tumor was successfully resected using a functional endoscopic endonasal approach, including sphenoethmoidectomy and maxillary sinus surgery.
  • Postoperative radiotherapy (72 Gy) was administered.
  • No local recurrence was observed up to 29 months post-surgery.

Implications:

  • Endoscopic resection combined with radiotherapy offers a viable treatment strategy for localized ethmoidal sinus leiomyosarcoma.
  • This approach may lead to favorable tumor control without orbital or skull base invasion.
