Summary
Long-term peritoneal dialysis (PD) can cause sclerosing peritonitis, a serious complication. Early detection and understanding of peritoneal membrane changes are crucial for patient management and developing preventive strategies.
Area of Science:
- Nephrology
- Pathology
Background:
- Peritoneal dialysis (PD) is a common treatment for end-stage renal disease.
- Long-term PD can lead to morphological and functional changes in the peritoneal membrane.
Observation:
- A patient on continuous ambulatory peritoneal dialysis (CAPD) for 7 months pre- and 86 months post-transplant experienced two peritonitis episodes.
- Post-hemodialysis, the patient developed ascites, GI disturbances, and an inflammatory syndrome, diagnosed as sclerosing peritonitis.
Findings:
- Sclerosing peritonitis was diagnosed after excluding other causes.
- The patient was treated with corticosteroid therapy.
Implications:
- This case highlights the peritoneal changes associated with long-term PD.
- Understanding pathophysiological mechanisms can guide the development of preventive measures, including more biocompatible dialysates.
