Infective endocarditis, a serious heart infection, can cause severe brain damage and neurological complications. This case highlights the rapid and fatal progression of Staphylococcus aureus endocarditis, even with prompt treatment.

Background:

  • Infective endocarditis frequently leads to severe neurological complications.
  • Congenital heart disease is a risk factor for developing endocarditis.

Observation:

  • A patient with Staphylococcus aureus endocarditis secondary to congenital heart disease presented with subacute neurological complications.
  • Despite prompt antibiotic therapy and negative blood cultures, the patient experienced rapid neurological deterioration.

Findings:

  • Neuropathological examination revealed extensive brain lesions including intraparenchymal hemorrhages, mycotic aneurysms, micro-abscesses, and septic arteritis with infarction.
  • Immunocytochemistry showed increased CD45 and GFAP, with adenosine A1 receptor expression on macrophages and microglia.
