Summary

Malignant struma ovarii, a rare ovarian tumor, can be diagnosed before surgery. This case highlights the importance of considering thyrotoxicosis in ovarian cancer evaluations.

Background:

  • Malignant struma ovarii is a rare ovarian neoplasm.
  • Preoperative diagnosis is challenging due to disease rarity and thyrotoxicosis complications.

Observation:

  • A 48-year-old woman presented with hyperthyroidism symptoms and a pelvic mass.
  • Elevated thyroid function tests were noted despite a normal-appearing thyroid gland.

Findings:

  • Preoperative diagnosis of hormone-producing malignant struma ovarii was established.
  • Surgical findings confirmed FIGO stage Ia ovarian papillary adenocarcinoma of the thyroid.
  • Positive thyroglobulin immunohistochemistry and high ovarian venous thyroglobulin levels were observed.

Implications:

  • Malignant struma ovarii is diagnosable preoperatively.
  • Thyrotoxicosis should be considered in the differential diagnosis of ovarian tumors.
