Introduction To Health Care Delivery System01:18

Introduction To Health Care Delivery System

The healthcare system is constantly changing and complex. Various services are available from different healthcare providers, but gaining access to these services has become challenging for people with limited healthcare insurance. Uninsured people present a challenge to healthcare because they frequently postpone or forego treatment.
The Institute of Medicine (IOM) advocates for a patient-centered, effective, safe, timely, equitable, and effective healthcare system. The National Priorities...

Barriers to Effective Communication II

The barriers to effective communication also include cultural barriers, semantic barriers, gender barriers, and time constraints.
Cultural barriers:
Differences in values, beliefs, religion, knowledge, and tradition can significantly impact communication. Awareness of nonverbal cues is critical, especially when conversing with a patient from a different culture. What appears appropriate in one culture may be inappropriate in another.

Introduction to Documentation and Reporting

Documentation is the systematic process of formally recording, maintaining, and communicating information.
Nursing documentation records essential information and details regarding a patient's care and treatment in written or electronic form. It is a critical aspect of nursing practice that involves documenting assessments, interventions, outcomes, and other relevant details about a patient's health status.
Documentation maps the patient's health journey by creating a comprehensive and precise...

Methods of Documentation VI: Case Management Model

The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic illness...

Area of Science:

  • Health Services Research
  • Health Disparities
  • Medical Interpretation

Background:

  • Limited-English-proficient (LEP) patients face significant barriers to accessing healthcare.
  • Effective communication is crucial for equitable healthcare delivery.
  • Professional interpreter services aim to bridge language gaps in clinical settings.

Purpose of the Study:

  • To evaluate the impact of professional interpreter services on healthcare utilization among LEP patients.
  • To determine if interpreter services increase the delivery of preventive and clinical healthcare services.
  • To assess the reduction in healthcare disparities after implementing interpreter services.

Main Methods:

  • A two-year retrospective cohort study was conducted in a New England Health Maintenance Organization (HMO).
  • Professional interpreter services were introduced between year one and year two for Portuguese and Spanish-speaking patients.
  • Data on clinical and preventive service use were extracted from computerized medical records for 4,380 adult patients.

Main Results:

  • Healthcare utilization, including office visits and prescription fulfillment, increased significantly more in the interpreter service group (ISG) compared to the comparison group (CG).
  • Receipt of preventive services, such as rectal examinations, also saw a significant increase in the ISG.
  • Disparities in fecal occult blood testing, rectal exams, and flu immunization rates were significantly reduced for LEP patients after interpreter service implementation.

Conclusions:

  • Professional interpreter services are effective in increasing the delivery of healthcare to limited-English-proficient patients.
  • Implementing interpreter services can help reduce existing healthcare disparities.
  • Investing in professional interpreter services is a viable strategy for improving health equity.
