A progressive goitre caused breathing difficulties and venous congestion, diagnosed by Pemberton's sign. Surgical removal of the thyroid gland completely resolved these symptoms.

Area of Science:

  • Endocrinology
  • Thoracic Surgery

Background:

  • Euthyroid goitres can progressively enlarge, potentially causing compressive symptoms.
  • Pemberton's sign is a clinical indicator of thoracic inlet obstruction.

Observation:

  • A 32-year-old woman presented with a progressive, non-toxic goitre and significant dyspnoea.
  • Positive Pemberton's sign was noted, characterized by inspiratory stridor and venous congestion upon bilateral arm elevation.

Findings:

  • The patient underwent a total thyroidectomy.
  • Post-operative resolution of dyspnoea, inspiratory stridor, and venous congestion was observed.

Implications:

  • Total thyroidectomy is an effective treatment for symptomatic euthyroid goitres causing thoracic inlet compression.
