Summary

Dural arteriovenous fistulas (DAVF) involving venous sinus occlusions can cause progressive dementia and white matter changes. Treatment of DAVF reversed these neurological deficits and MRI findings.

Background:

  • Dural arteriovenous fistulas (DAVF) are abnormal connections between dural arteries and veins.
  • Progressive dementia can arise from various neurological conditions, including white matter diseases.

Observation:

  • Two patients presented with progressive dementia and diffuse white matter changes on MRI.
  • Cerebral angiography revealed DAVF involving transverse-sigmoid sinuses with associated dural venous sinus occlusive changes.

Findings:

  • Occlusive changes isolated the straight sinus, concentrating DAVF drainage and causing venous hypertension.
  • This venous hypertension led to venous ischemia of the white matter, manifesting as diffuse changes on MRI and neurological decline.

  • DAVF with venous sinus occlusions is a potential cause of dementia with diffuse white matter changes.
  • Treatment of DAVF can reverse white matter abnormalities and neurological symptoms, highlighting the importance of timely diagnosis and intervention.
