    Summary

    Phenylbutazone administration decreases total warfarin levels but increases unbound warfarin. This occurs because phenylbutazone inhibits the metabolism of S-warfarin, leading to enhanced anticoagulation.

    Background:

    • Warfarin is a widely used anticoagulant.
    • Phenylbutazone is an anti-inflammatory drug.

    Purpose of the Study:

    • To investigate the pharmacokinetic interaction between phenylbutazone and warfarin.
    • To determine the effect of phenylbutazone on warfarin disposition and metabolism.

    Main Methods:

    • A single-subject study design was employed.
    • Warfarin was administered daily for one month.
    • Phenylbutazone was co-administered to assess its impact on warfarin pharmacokinetics.

    Main Results:

    • Total plasma warfarin concentration decreased from 4.2 to 2.0 mg/L during phenylbutazone administration.
    • Unbound warfarin plasma concentration increased from 0.020 to 0.033 mg/L.
    • Plasma levels and urinary excretion of 7-hydroxywarfarin, a metabolite of S-warfarin, decreased.

    Conclusions:

    • Phenylbutazone inhibits the metabolism of the more potent S-warfarin isomer.
    • This inhibition of S-warfarin metabolism explains the increased anticoagulation observed when phenylbutazone is added to warfarin therapy.
