Summary

Intraluminal thrombus formation occurs in nearly 25% of patients after aortic endograft repair. This study investigated thrombus incidence and characteristics following endovascular aneurysm repair.

  • Endovascular aneurysm repair (EVAR) is a common treatment for aortic aneurysms.
  • Intraprosthetic complications can affect long-term outcomes.
  • Thrombotic deposit formation within aortic endografts requires further investigation.

Purpose of the Study:

  • To determine the incidence of intraprosthetic thrombotic deposits in patients who underwent aortic endograft implantation.
  • To characterize the location and timing of thrombus formation within aortic endografts.

Main Methods:

  • Retrospective analysis of 82 patients undergoing transfemoral implantation of aortic stent-grafts.
  • Three-phase helical computed tomographic (CT) examinations performed at 3-month intervals.

  • Main Results:

    • Intraluminal thrombotic deposits were observed in 19 (23.2%) patients over a mean follow-up of 12.1 months.
    • Thrombus formation was most common in infrarenal, bifurcated endografts, appearing on average 7 months postoperatively.
    • Non-occlusive thrombus occurred in the aortic segment (16 patients) or graft limbs (5 patients); 3 cases showed complete thrombus resolution.

    Conclusions:

    • Nearly one in four patients may develop intraluminal thrombus after endovascular aortic aneurysm treatment.
    • The influence of endograft geometry and intraprosthetic flow dynamics on thrombus deposition warrants future research.
