Summary

Topiramate shows promise as a mood stabilizer for bipolar I disorder, significantly reducing hospitalizations in one patient. This anticonvulsant was well-tolerated and improved long-term functioning.

Background:

  • Bipolar disorder, specifically Bipolar I, is characterized by recurrent manic episodes.
  • Previous treatments including carbamazepine, valproate, and lamotrigine showed limited efficacy in this patient.
  • Topiramate, an anticonvulsant with a broad mechanism of action, has emerging evidence for mood-stabilizing properties.

Observation:

  • A 41-year-old female patient with a 10-year history of Bipolar I disorder and 12 prior hospitalizations was treated with topiramate.
  • Initial treatment led to a brief hospitalization, but subsequent management with adjusted topiramate dosage and adjunctive medications (risperidone, clonazepam) prevented further hospitalizations.
  • The patient experienced a recurrence of psychopathology following a significant life event, which was managed without inpatient care.

  • The patient remained largely asymptomatic on a maintenance dose of 200 mg/day of topiramate.
