    Summary

    Elevated serum Immunoglobulin-E (IgE) levels are common in atopic dermatitis and atopic respiratory disease patients. However, high IgE in patients with both conditions doesn't correlate with disease severity or skin test reactivity.

    Background:

    • Atopic diseases, including atopic dermatitis (AD) and atopic respiratory disease (ARD), are characterized by heightened immune responses.
    • Serum Immunoglobulin-E (IgE) is a key marker in allergic inflammation, but its precise correlation with disease phenotypes requires further investigation.

    Purpose of the Study:

    • To investigate the relationship between serum IgE levels and clinical manifestations in patients with atopic diseases.
    • To compare IgE levels and skin test reactivity in patients with AD, ARD, and combined AD-ARD.

    Main Methods:

    • Study included 57 subjects: 43 with atopic diseases (ARD, AD, or both) and 14 non-atopic controls.
    • Utilized immediate skin test allergens and radio-immunoassay for serum IgE quantification.
    • Analyzed geometric mean serum IgE levels and correlated them with disease activity and skin test results.

    Main Results:

    • Serum IgE levels were significantly elevated in ARD (170 units/ml), AD (320 units/ml), and combined AD-ARD (772 units/ml) patients compared to controls (50 units/ml).
    • The highest IgE levels in AD-ARD patients were not linked to respiratory disease activity or extent of dermatitis.
    • No overall correlation was found between serum IgE levels and the frequency of positive skin tests; AD patients had higher IgE but lower skin test reactivity than ARD patients.

    Conclusions:

    • Patients with combined AD-ARD exhibit exceptionally high serum IgE, suggesting a 'highly atopic' phenotype.
    • Serum IgE levels do not consistently correlate with disease severity or allergen-specific skin test responses in atopic individuals.
    • AD patients demonstrate a dissociation between elevated serum IgE and reduced skin test reactivity to common allergens.
