The modified HAI and Scheuer classifications effectively evaluate histologic changes in patients with chronic hepatitis C who respond to interferon therapy. However, predicting treatment response using these systems remains challenging.

- Patients were categorized into complete responders (n=15), partial responders (n=24), and nonresponders (n=7).

- Complete responders showed significant improvement in grade and stage with modified HAI and Scheuer classifications.
- The original HAI showed improvement in grade but not stage in complete responders.
- No significant changes were observed in partial responders or nonresponders across all systems.
- Pretreatment piecemeal necrosis scores and Scheuer fibrosis scores predicted nonresponse.
Conclusions:
- The modified HAI and Scheuer classification are useful for evaluating histologic changes in complete responders to IFN therapy.
- Predicting complete response prior to treatment was not feasible with any system.
- Specific histologic features, like piecemeal necrosis and fibrosis, can indicate nonresponse to IFN therapy.
