Area of Science:

  • Nephrology
  • Geriatrics
  • Internal Medicine

Background:

  • Increasing number of elderly patients require dialysis for end-stage renal failure.
  • Limited data exists on optimal dialysis therapy for patients aged 80 and above.
  • Focus on the 'old old' demographic highlights a critical gap in geriatric nephrology.

Purpose of the Study:

  • To evaluate the safety and suitability of chronic peritoneal dialysis (CPD) in patients aged 80 years and older.
  • To analyze patient and technique survival, complications, and treatment outcomes in this specific elderly population.
  • To provide evidence-based recommendations for managing end-stage renal failure in the 'old old'.

Main Methods:

  • Retrospective analysis of 31 non-institutionalized and 7 institutionalized 'old old' patients.

Related Experiment Videos

  • Patients were treated with Continuous Ambulatory Peritoneal Dialysis (CAPD) or Continuous Cycler-Assisted Peritoneal Dialysis (CCPD).
  • Data collected included patient/technique survival, laboratory results, and dialysis-related complications.
  • Main Results:

    • Low rates of peritonitis (1/28.6 patient-months) and exit-site infections (1/75.1 patient-months) were observed in non-institutionalized patients.
    • Hospitalization incidence was 1/14.7 patient-months, with an average stay of 7.5 days/patient-year.
    • Forty-seven percent of patients survived 24 months, and technique survival was 91.5% at 12 months; malnutrition was common.

    Conclusions:

    • Chronic peritoneal dialysis is a safe and suitable treatment option for end-stage renal failure in 'old old' patients.
    • The modality demonstrates acceptable survival rates and manageable complication profiles in this elderly cohort.
    • Further research may explore strategies to mitigate malnutrition and optimize care for this growing patient group.
