Area of Science:

  • Neurology
  • Radiology
  • Infectious Diseases

Background:

  • Brain lymphoma is an increasing complication in Acquired Immunodeficiency Syndrome (AIDS) patients.
  • Highly Active Antiretroviral Therapy (HAART) has prolonged life expectancy for AIDS patients, leading to more late-stage complications.
  • Focal brain lesions are common in AIDS patients, requiring accurate diagnosis.

Purpose of the Study:

  • To evaluate the diagnostic utility of brain Single-Photon Emission Computed Tomography (SPECT) using Thallium-201 (201Tl) in AIDS patients with focal brain lesions.
  • To assess SPECT's role in the differential diagnosis of cerebral lesions in this population.

Main Methods:

  • Seventeen AIDS patients with neurological symptoms and focal central nervous system (CNS) lesions on CT were studied.

Related Experiment Videos

  • SPECT images were analyzed for focal tracer uptake compared to adjacent tissues.
  • Results were correlated with serology, clinical outcomes, imaging follow-up, and biopsy data.
  • Main Results:

    • SPECT revealed focal radiotracer uptake in 3 patients, all of whom had negative toxoplasmosis serology and died soon after.
    • Four patients were diagnosed with progressive multifocal leukoencephalopathy (PML).
    • Ten patients showed good clinical and/or radiological improvement, suggesting other diagnoses.

    Conclusions:

    • Brain SPECT with 201Tl is a valuable non-invasive method for differentiating cerebral focal lesions in AIDS patients.
    • The technique aids in identifying specific conditions and guiding patient management.
