Area of Science:

  • Endocrinology
  • Immunology
  • Reproductive Medicine

Background:

  • Postpartum thyroiditis (PPT) is an autoimmune thyroid disorder occurring within the first year after childbirth.
  • It typically presents with transient thyrotoxicosis and hypothyroidism, potentially leading to permanent hypothyroidism.

Purpose of the Study:

  • To determine the prevalence of postpartum thyroiditis (PPT) in Tehranian women.
  • To investigate the clinical and serological characteristics of PPT.

Main Methods:

  • A cohort of 1040 mothers was studied across five health centers in Tehran.
  • Thyroid function tests (T3, T4, TSH), autoantibodies (anti-TPO, anti-Tg), and thyroid sonography were performed at multiple time points postpartum.
  • Clinical signs of thyroid dysfunction and goiter were assessed.

Related Experiment Videos

Main Results:

  • The prevalence of postpartum thyroiditis was found to be 11.4%.
  • Hypothyroidism and thyrotoxicosis were observed in 68 and 42 mothers, respectively, with 9 cases exhibiting a biphasic pattern.
  • Elevated anti-TPO and anti-Tg antibodies, along with sonographic changes, were significantly more common in patients with PPT compared to controls (P<0.001).
  • Goiter (Stage II) was also significantly more prevalent in the PPT group (21.8% vs 6.7%, P<0.001).

Conclusions:

  • Tehranian women exhibit a high prevalence of postpartum thyroiditis.
  • The observed high prevalence may be linked to increased follow-up frequency or a transition in iodine intake levels.
  • A lower frequency of the biphasic form of PPT was noted compared to other studies.
