Background:

  • Primary pulmonary plasmacytoma is an exceptionally rare plasma cell neoplasm.
  • Diagnosis is often challenging due to nonspecific symptoms and rarity.

Observation:

  • An elderly patient presented with dyspnea, cough, weight loss, and malaise.
  • Chest radiography revealed diffuse pulmonary infiltration.
  • Open lung biopsy showed infiltration by well-differentiated plasma cells and amyloid deposits.

Findings:

  • Immunohistochemistry confirmed plasma cells and amyloid expressing a monoclonal lambda light chain.
  • Serum and urine immunoelectrophoresis did not show a monoclonal spike.
  • Skeletal survey and bone marrow biopsy ruled out disseminated disease, confirming extramedullary plasmacytoma.

Implications:

  • Extramedullary plasmacytoma of the lung can be treated effectively with chemotherapy.
  • This case demonstrates a favorable long-term outcome with complete remission.
  • Highlights the importance of immunohistochemistry in diagnosing rare pulmonary neoplasms.
