Summary

Complex HIV-1 therapies did not outperform standard triple therapy in lymph nodes. Viral persistence in lymph nodes may stem from cellular resistance, not drug potency. This study tracked HIV-1 RNA in lymph nodes over 78 weeks.

Purpose of the Study:

  • To evaluate the impact of a five-drug combination therapy on HIV-1 RNA levels within lymph nodes.
  • To assess the effectiveness of subsequent four- and three-drug maintenance regimens.
  • To analyze HIV-1 RNA dynamics in lymph nodes and correlate with plasma viremia and immune markers.

Main Methods:

  • Ten treatment-naive patients received sequential five-, four-, and three-drug antiretroviral therapy over 78 weeks.
  • HIV-1 RNA in lymph nodes was quantified using PCR at multiple time points.
  • Plasma HIV-1 RNA, PBMC proviral DNA, lymphocyte subsets, and drug levels were monitored.

Main Results:

  • Eight of ten patients achieved stable, undetectable HIV-1 RNA in lymph nodes with maintenance regimens.
  • A mean log drop of -3.17 in lymph node HIV-1 RNA was observed in responders.
  • One patient experienced viral rebound due to non-adherence; another showed stable lymph node RNA with specific mutations and low drug levels.

Conclusions:

  • Intensified combination therapy offers no superior benefit in lymph nodes compared to standard triple therapy.
  • Persistent HIV-1 RNA in lymph nodes may be attributed to cellular resistance mechanisms.
  • Further research into cellular factors influencing HIV-1 persistence in lymph nodes is warranted.
