Background:

  • Percutaneous transluminal coronary angioplasty (PTCA) for chronic total occlusions (CTO) has limited success.
  • Guidewire navigation failure is a primary cause of PTCA failure in CTO.
  • Novel guidance systems are needed to improve CTO intervention outcomes.

Purpose of the Study:

  • To evaluate a prototype Optical Coherent Reflectometry (OCR) guidewire for navigating chronic total arterial occlusions.
  • To determine if OCR can differentiate between various plaque compositions and tissue layers.
  • To assess the accuracy of OCR in determining guidewire position within occluded arteries.

Main Methods:

  • Developed a 0.014" hypotube guidewire incorporating an OCR fiber optic (1,300 nm wavelength).
  • Analyzed OCR reflectance slopes to differentiate human arterial plaque types (calcified, white, yellow) in vitro.
  • Compared OCR-based guidewire positioning with intravascular ultrasound (IVUS) in occluded human artery segments.

Main Results:

  • OCR demonstrated a significantly steeper reflectance slope for calcified plaques compared to non-calcified ones (P < 0.01).
  • OCR and IVUS showed 82% agreement in determining guidewire position within occluded arteries.
  • OCR achieved 79% sensitivity and 89% specificity in distinguishing plaque from the media/adventitia boundary (P < 0.001).

Conclusions:

  • Optical Coherent Reflectometry can effectively differentiate calcified from non-calcified arterial plaque.
  • OCR shows promise in distinguishing plaque from deeper arterial wall layers.
  • This OCR guidewire technology may enhance safe navigation during interventions for chronic total occlusions.
